{"id":2317,"date":"2015-01-15T15:57:33","date_gmt":"2015-01-15T20:57:33","guid":{"rendered":"http:\/\/www.ptca.org\/voice\/?p=2317"},"modified":"2015-01-15T15:57:33","modified_gmt":"2015-01-15T20:57:33","slug":"nbpas-launches-alternative-to-moc","status":"publish","type":"post","link":"http:\/\/www.ptca.org\/voice\/2015\/01\/15\/nbpas-launches-alternative-to-moc-2317","title":{"rendered":"NBPAS Launches Alternative to MOC"},"content":{"rendered":"<p><img loading=\"lazy\" decoding=\"async\" class=\"alignleft size-full wp-image-2318\" src=\"http:\/\/www.ptca.org\/voice\/wp-content\/uploads\/2015\/01\/NBPAS_logo.jpg\" alt=\"   \" width=\"140\" height=\"140\" \/>Today&#8217;s the day that <a title=\"The National Board of Physicians and Surgeons (NBAPS)\" href=\"http:\/\/nbpas.org\/\" target=\"_blank\">The National Board of Physicians and Surgeons (NBPAS)<\/a> officially launches. Briefly stated, it&#8217;s an alternative certification organization, set up by Dr. Paul Teirstein and a group of physicians, mostly cardiologists, who strongly objected to the Maintenance of Certification assessment track established by the American Board of Internal Medicine (ABIM).<\/p>\n<p>You can read more about it in my post from last week, &#8220;<a title=\"Scalpel\u2026Suture\u2026Suction\u2026Pencil?\" href=\"http:\/\/www.ptca.org\/voice\/2015\/01\/08\/scalpel-suture-suction-pencil-2295\" target=\"_blank\">Scalpel\u2026Suture\u2026Suction\u2026Pencil?<\/a>&#8221; and in Dr. Teirstein&#8217;s <em>New England Journal of Medicine <\/em>article<em>, <\/em>titled \u201c<a title=\"Boarded to Why Maintenance of Certification Is Bad for Doctors and Patients\" href=\"http:\/\/www.nejm.org\/doi\/full\/10.1056\/NEJMp1407422\" target=\"_blank\">Boarded to Death \u2014 Why Maintenance of Certification Is Bad for Doctors and Patients.<\/a>\u201d<\/p>\n<p>I&#8217;ll be following the progress of this initiative, but a few recent articles and developments bring up some interesting points.<!--more--><\/p>\n<p>One very vocal critic of the MOC process has been cardiologist Dr. Westby Fisher. And while Dr. Wes admires Dr. Teirstein&#8217;s efforts, <a title=\"Some Thoughts on the National Board of Physicians and Surgeons \" href=\"http:\/\/drwes.blogspot.com\/2015\/01\/some-thoughts-on-national-board-of.html\" target=\"_blank\">he has reservations<\/a> about the possibility of success for an organization, concluding instead:<\/p>\n<blockquote><p>&#8220;In my view the only way to truly &#8216;change&#8217; MOC is to have a coordinated effort from all specialty societies to insist our legislators remove the portion of our new health care law that requires we participate in a &#8216;physician registry&#8217; that robs not only practicing physicians, but patient care itself.&#8221;<\/p><\/blockquote>\n<p>A fact of the MOC process is that it was written into the Affordable Care Act and, as such, has become a de facto requirement for physicians. Even though a physician is &#8220;Board Certified,&#8221; something that is revisited every ten years, the MOC process now requires the fulfillment of certain requirements every two years, activities which the 21,000 physicians who signed Teirstein&#8217;s petition view as &#8220;complex, have questionable value, and detract from more worthwhile pursuits, including patient care and other educational activities.&#8221; If a physician does not complete these activities, he\/she is labeled, &#8220;certified, not meeting MOC requirements,&#8221; and that doctor&#8217;s employer\/hospital system will most likely deem that unacceptable.<\/p>\n<p>Dr. Wes has company. A number of physician-bloggers have chimed in as critics of the MOC process, for example, Dr. John Mandrola whose blog post, &#8220;<a title=\"A scary new medical intervention\u2026\" href=\"http:\/\/www.drjohnm.org\/2015\/01\/a-scary-new-medical-intervention\/\" target=\"_blank\">A scary new medical intervention\u2026<\/a>,&#8221; details his objections. And Dr. Seth Bilazarian in his article, &#8220;<a title=\"Board Recertification: There Has to Be a Better Way\" href=\"http:\/\/www.medscape.com\/viewarticle\/837548\" target=\"_blank\">Board Recertification: There Has to Be a Better Way<\/a>.&#8221;<\/p>\n<p>Another strong supporter of Dr. Teirstein is Dr. Robert Harrington, Professor and Chair, Department of Medicine, Stanford University, a regular contributor to <a title=\"theheart.org-Medscape\" href=\"http:\/\/www.medscape.com\/cardiology\" target=\"_blank\">theheart.org-Medscape<\/a> (as are Drs. Mandrola and Bilazarian), and the Committee Chair for the past two Annual Scientific Symposia of the American Heart Association. He commented to me earlier today:<\/p>\n<blockquote><p>&#8220;I&#8217;m hugely supportive and appreciative of Paul&#8217;s leadership in this area. \u00a0This is a critically important time in medicine for leaders to address how best to assure lifelong competency in clinicians so that the public can be confident in the profession&#8217;s ability to keep current with a rapidly evolving knowledge base.&#8221;<\/p><\/blockquote>\n<p>However, a cautionary note, posted by Dr. Edward J Schloss on <a title=\"Edward J. Schloss' Twitter Feed\" href=\"https:\/\/twitter.com\/EJSMD\/status\/554282734646865921\" target=\"_blank\">his Twitter feed<\/a>, refers to Senator Rand Paul&#8217;s attempt to establish <a title=\"Rand Paul has another problem - Washington Post\" href=\"http:\/\/www.washingtonpost.com\/blogs\/right-turn\/wp\/2013\/11\/08\/rand-paul-has-another-problem\/\" target=\"_blank\">a new certification organization for ophthalmology<\/a> back in 2010. Without the backing of a national professional organization, any certification board may not have the ability to convince hospitals, insurers, government agencies, and, oh yes, PATIENTS, that its imprimatur has meaning. I would point out, however, that Senator Paul&#8217;s organization consisted of himself, his wife and his father-in-law. The National Board of Physicians and Surgeons has recruited, as mentioned, 21,000 to sign a petition criticizing the MOC, and almost 7,000 physicians pledging to boycott the ABIM requirements.<\/p>\n<p>Perhaps the launch and strong support of Dr. Teirstein&#8217;s alternative will have the effect of bringing the professional organizations to the table and working with the ABIM to modify and &#8220;smarten up&#8221; these regulations, something posited by Dr. Westby Fisher. To that end, and very possibly in response to the unrest noted above, the American College of Cardiology just issued a &#8220;<a title=\"Follow-Up on ABIM Maintenance of Certification\" href=\"http:\/\/content.onlinejacc.org\/article.aspx?articleID=2089103\" target=\"_blank\">Follow-Up on ABIM Maintenance of Certification<\/a>&#8221; in this month&#8217;s JACC, &#8220;intended to update the ACC membership on progress in our efforts to encourage the\u00a0ABIM to modify the MOC process.&#8221; Several meetings between the ACC and ABIM have been taking place and this month over a dozen new members were appointed to the ABIM cardiology board &#8220;<span id=\"scm6MainContent_rptSections_lblSection_20\" class=\"Paragraph 70068055\">following a broad canvas of stakeholder recommendations, including those of the ACC.&#8221;<\/span><\/p>\n<p>Many questions, but one thing for sure: this is definitely an issue that will not be going away anytime soon.
